The above documents are in Microsoft Word - Template forms or PDF format.  To use the Word Templates, save and open the document, double click on the first fill in (it may be highlighted when you open the document), enter the correct information, then use the tab key to navigate to the next fill in.  After you have completed the fill in portions, print the document, sign and return to address on the form. The Simple Development Plan Example is a PDF document, and you must have Acrobat Reader installed on your machine! It is free all you need to do is follow this link and install it.
